On November 20, 2025, Licensing Program Analyst (LPA) Ivan Avila arrived at the facility to conduct a case management visit. LPA met with Program Manager Cole Militano and explained the purpose of the visit.

LPA and Program Manager discussed documents regarding a employee transfer request.

No deficiencies cited during today's inspection. Exit interview conducted and copy of report was provided.
