Facility Evaluation Report

On October 29, 2025, Licensing Program Analyst (LPA) Ivan Avila arrived unannounced at the facility to conduct a case management visit regarding an incident report the department received on October 23, 2025. LPA met with supervisor Maria Marquez and explained the purpose of the visit.

LPA addressed the incident that occurred on October 23, 2025, at approximately 1:30 PM. Client eloped from the Day Program through the front door while staff immediately followed the client. Staff followed the client down the street and proceeded to provide crisis communication while ensuring the client’s safety. The client continued to walk down the street to a busy road and staff tried to re-direct the client but had immediately dropped to the ground. The client became aggressive and tried to scratch and bite staff. At this time local law enforcement saw the incident and was informed of the current situation. The police officer assessed the incident as the client was in an unsafe environment and placed the client in the day program’s company vehicle where the client was taken back to the day program. While being placed in the company vehicle the client was combative toward the police officer. At the day program the client did not want to get out of the vehicle until their home provider arrived and was able to talk to the client. The client got out of the vehicle and then taken home by their provider.

Staff followed company policy and procedures to ensure the health and safety of the client. LPA spoke to staff about the importance of client safety in and out of the facility.

No deficiencies cited. Exit interview conducted. A copy of this report was provided.
