Facility Evaluation Report

On 2/16/2022, around 4:00pm, Licensing Program Analyst (LPA) Mai Thao arrived at the facility unannounced to conduct a Plan of Correction (POC) visit. LPA met with Manpreet Dyal, Co-Licensee and explained the purpose of the visit. Prior to initiating the visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N95.

Deficiency cited under Title 22 Regulations have been cleared. Licensee complied with the terms of the POC by POC due date.

Facility was provided POC cleared letter.

Exit interview conducted and a copy of this report was left at the facility.
