The inspector’s account

LPA Hiratsuka conducted this unannounced case management visit in response to an incident that occurred on August 23, 2023. A resident left the facility without permission. LPA reviewed the resident's physician's report and it stated the resident may leave without assistance. LPA also obtained more information about the resident during today's visit.
