Facility Evaluation Report

Licensing Program Analyst K. Hiratsuka conducted this unannounced annual visit. LPA met toured with Caregiver Erika Hayes. Administrator Lata Rani, arrived during visit and met with LPA.

This facility has a fire clearance for six non-ambulatory residents. There are two shared and two private resident rooms. There is one staff room. The staff room has a full private bathroom. There is one full common bathroom that has both a shower and tub. There is a common area that connects to the kitchen and dining area. There is an office/activity area behind the dining room that has a locked cabinet for medications and files. The backyard has gates on both sides. There are two locked sheds in the backyard. There is a laundry room that connects to the garage.
