Facility Evaluation Report

Licensing Program Analyst (LPA) Hiratsuka conducted this unannounced visit in response to an incident the facility reported.

On 06/02/2025, the facility administrator reported a resident left the facility without permission from the facility. The physician's report has a section regarding if a resident is able to leave the facility unassisted and the physician is supposed to check yes or no. The physician's report had both boxes checked with one that was marked with error. Today, LPA was able to clarify that the resident is able to leave the facility unassisted. The resident has not returned to the facility as of today; however, during this visit Administrator received a phone call that the resident was found.

Based on the above, no deficiencies cited.
