Facility Evaluation Report
Licensing Program Analyst (LPA) conducted this unannounced annual visit. LPA toured the facility with Administrator Rhea Dodd.
During the inspection LPA toured facility inside and out with Administrator to ensure health and safety of clients in care. Areas toured include but are not limited to: common area, office area, four resident bedrooms, one shared bathroom, laundry room, kitchen, dining area, and storage areas. In the areas toured no immediate health, safety, or personal rights violations were observed. LPA and administrator completed the infection control domain and facility was found to be in substantial compliance at this time.
LPA reviewed two staff and two resident records.
No deficiencies are being cited as a result of today’s inspection.
Exit interview conducted and copy of this report was left at the facility.
