The inspector’s account

Licensing Program Analyst (LPA) Hiratsuka conducted this unannounced case management visit.

This visit in response to the multiple incidents regarding one resident. The resident is currently not in the facility and there are no plans for the resident to return.

LPA interviewed two of the three residents. LPA also interviewed Assistant Erin Duchene.
