Licensing Program Analyst (LPA) Hiratsuka conducted this unannounced case management visit. LPA spoke with Assistant Administrator Erin Duchene.

This is follow-up visit to a resident who was moved out earlier this month. The former resident had a lot of incidents both at this facility and in the community.
