Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi and Lavinia Muscan arrived unannounced to conduct an annual inspection. LPA met with administrator Vanessa Rodriguez during today's inspection. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: Masks.

During today's inspection LPA's were following up on a DDS visit conducted in February 2023. LPA reviewed client records, documentation, infection control plans, and toured the facility. LPA observed a doctor summary note that had white out on the form. Administrator will provide LPA with the summary visit documentation and any other follow physician documentation from that visit.

No deficiencies cited during today's inspection.
