Licensing Program Analyst Hiratsuka, conducted this unannounced case management visit.
The beginning of May 2026, LPA called and requested a copy of the facility's initial application that they submitted in June 2020. LPA emailed the request to Ms. Mireles several times who stated she was informed each time the paperwork was found and going to be sent over. As of today LPA has not received the paperwork..
The licensee has until June 5, 2026, to submit the paperwork to Community Care Licensing Division. Failure to do so shall result in a deficiency being cited.
