The inspector’s account

Licensing Program Analyst (LPA) Hiratsuka, conducted this unannounced case management visit in response to a resident who left the facility without being noticed on July 19, 2025, and not noticed gone until the early morning of July 20, 2025.

Assistant Administrator (AA) stated they were able to determine the resident left out the front doors around 7:30pm on July 19, 2025, and the staff didn't notice until about 1:30am. The local sheriff's office was contacted and a search was conducted and the resident was found a couple of blocks away. The resident was sent to the hospital and has returned without ill effects. AA stated the staff did not conduct the checks they are assigned to do for residents.

AA stated there are now elopement drills for the staff over the next thirty days, the staff are to monitor and document they have checked on residents, and there are going to be further instructions based on resident needs to be given to staff.

Based on the above, the following citation was issued today from the California Health and Safety Code.

Deficiencies cited from Title 22 Regulations and or the California Health and Safety Code. Failure to correct shall result in civil penalties. appeal rights left.
