Licensing Program Analysts (LPAs) Hiratsuka and Avila, conducted this visit in response to incidents that involved one resident on and off the facility grounds. Administrator Leonard Willis contacted LPA Hiratsuka by phone on Monday, September 22, 2025, to report the incidents and followed-up with the written reports.
Today, LPAs interviewed staff and obtained documentation.
