The inspector’s account

Licensing Program Analyst, LPA, Noel Wolf Petersen arrived to the facility and met with the senior most staff Nichole Cole to explain the purpose of the visit: to continue to conduct a previously started annual inspection. Administrator Marisol is out for another 2 weeks.

The LPA met with the executive director, Chris Martin. Physically the main common area, front entryway, and client bathroom facility is observed to be clean, and the traffic areas are unobstructed and well lit. 3 Fire exits were observed chained or pinned, with clients in the building, there is still a front exit that is open. LPA gave guidance that the fire exits should be unobstructed while there are clients in the building.

A review of 2 client files showed up to date IPPs, health screenings, and signed admission agreement.

A review of 5 staff files included health screening, background check, continuing and initial training.

The LPA reviewed administrator docs, grievance policy, refund policy, workers comp, the initial training procedure.

No citations issued at this time, A copy of the report was read and left with the staff, an exit interview was conducted.
