Complaint Investigation Report
All clients interviewed by LPA Lund denied being yelled at or mistreated by S1 or any staff at the facility. S1 never lost any supervision of the clients or C1 during the incident. C1 did go into a neighbor’s backyard without permission and S1 was able to persuade C1 out the backyard and back to the facility along with other clients in care safely. The facility turned in Valley Mountain Special Incident report to Valley Mountain and CCL
The Department (CCLD) has found the allegations. Unsubstantiated.
A finding that the complaint allegation(s) are UNSUBSTANTIATED means that although the allegation(s) may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation(s) occurred
