Facility Evaluation Report
Licensing Program Analyst, LPA, Noel Wolf Petersen arrivied to the facility to conduct an annual inspection. LPA met with staff willard, and then later Administrator Tyrone Rush to explain the purpose of the visit.
A physical inspection was completed, including the bedrooms, bathrooms ,exterior, evacuation route gate. Facility is clean and the traffic areas are unobstructed. Bedrooms and bathrooms have required furniture and furnishings. bathroom temperature is measured at the sink between 105-120*F. common areas and exteriors are free of trip hazzards and have room for activites. evacuation route gate swings freely and latches closed.
fire extinguishers are dated. Jan, 21, 2026. smoke alarm is functional. first aid kit has all required items.
Resident files were reviewed, for 5 clients. Ipp and admission agreements and most recent medical assessment and original health screenings were present and up to date.
Staff files were reviewed, for 2 staff. health screenings and backfround clearance, ongoing trainings, firstaid/cpr were present and up to date.
Administrator files were reviewed, required postings, personal rights, Ombudsman, menu, activity log, federal workers rights, facility license, administrators certificate, evacuation plan, infection control plan, fire drill, and resident council meetings. files are present and up to date.
no citations as part of this visit. A copy of the report was read and given to the administrator. exit interview conducted.
