Facility Evaluation Report
Licensing Program Analyst (LPA) Renee Campbell arrived unannounced to conduct an annual inspection. LPA Campbell met with Tyrone Rush, Administrator and Victor Cardona, Regional Manager and explained the purpose of the visit. Three residents are present in the facility, two of which were observed by LPA Campbell.
LPA Campbell toured the facility and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas to ensure there are no safety hazards for residents. Furniture and furnishings were sufficient to meet the needs of residents. One resident (R1) had thrown various items that included a lamp. The facility therefore removed or did not replace breakable items such as ceramic or glass due to repeated incidents of angry outbursts.
The facility temperature was 71 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature measured 118 degrees Fahrenheit, which is within the required range of 105 and 120 degrees. The backyard was toured by LPA Campbell and pathways and exits were found to be clear. All screens were observed to be intact.
