Facility Evaluation Report
Licensing program Analyst (LPA) Renee Campbell conducted an unannounced annual required visit at the above Facility. LPA Campbell met with the Jessica Villaseno, Administrative Team Lead and toured the facility inside and outside.
The Paulson Community consists of three one story home style buildings built in a semi-circle with the main administrative building included. The Paulson-Yosemite building is licensed to serve 5 developmentally disabled residents, 5 of whom may be non-ambulatory. Residents are between the ages of 19 and 59 and the building is approved for delayed egress.
The facility has 1 common area, a staff bathroom and 5 resident rooms with a bathroom in each. There is a meeting area, dining area, and a laundry area that is kept locked. Each client has their own bathroom with grab bars, paper towels and soap. There are cameras on the outside of the building.
The Facility has a kitchen area with a sink, refrigerator and dishwasher. Knives are locked and inaccessible to residents. The medications were locked in the Medication Office. There is a delayed egress on the front door, side gate from the backyard and exterior gate in the common area of the facility.
There are designated cabinets for client’s belongings outside of each room. An ombudsman poster was observed in the medication office. Per the fire drill logs, fire drills are conducted successfully monthly with staff and residents. At that time, staff will do a headcount, check the location of fire extinguishers, meds and van keys.Exit interview was held with the Administrator and a copy of report given at the conclusion of the visit.
