Facility Evaluation Report

On 06/17/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a closure visit.

Upon arrival to this facility, LPA was unable to gain entry to the facility due to no one being present at this time. It was learned that all residents whom resided at the facility have moved out into other licensed care facilities on 06/02/2025. LPA walked around the perimeter of the facility and observed that the window shades were shut, no cars parked in the drive way, and overgrown weeds. In addition, there was indication that there are currently any residents who require care and supervision at this time.

This facility license will be closed out as of this date, 06/17/2025.

A closure letter as well as a copy of this report will be sent to the facilities mailing address.
