Facility Evaluation Report
Licensing Program Analysts (LPAs) Jason Lund & Charlie Yang arrived unannounced to conduct a Health and Safety Visit. LPA met with House Manager (HM), Alicia Contreras and explained the purpose of the visit. The purpose of this visit was to ensure the health and safety of residents.
Current census was 6. A brief interview with HM Alicia Contreras was conducted, The facility has been told on and given documentation on how to relocate the residents in care.
LPAs inspected the facility; food supply was reviewed for adequate 2-day perishable and 7-day nonperishable quantities at all times. Food storage unit, refrigerator, was reviewed in the kitchen area. Cabinets were observed to store non perishable food items and dry goods at this time. Tour of the resident bedrooms was conducted. Resident bedroom furniture and furnishings were observed to be sufficient to meet the needs of the residents at this time. Tour of the resident restrooms was conducted. Grab bars and non-skid mats were observed to be present and in good repair at this time. A tour of the exterior grounds was conducted. A review of the facility perimeter fence, side gates, and exits was conducted.
The department will continue to monitor the situation with health and safety checks at this time.
Based on the observations made during this visit, there are no deficiencies cited during this visit. An exit interview was conducted and a copy of this report was provided to the facility at the end of this visit.
