Facility Evaluation Report

On 05/08/2025, Licensing Program Analyst (LPA) Renee Campbell arrived to the facility unannounced in response to the regional office being notified that the administrator and owner, Kelsey Ramos, had passed. LPA Campbell met with Cindy Fuentes, Caregiver , and explained the reason for the visit.

LPA Campbell observed that the facility was free of odor and pathways were clear. There were 5 residents present in the facility. There was 1 staff present. LPA Campbell asked staff if there was an update regarding contacting powers of attorney for the residents and if the facility needed anything to help assist residents. Staff stated she had not heard about contacting families.

Caregiver Fuentes stated that the facility continued to be operated as needed and that there was no change in the residents level of care LPA Campbell requested and received the facility roster with the residents family and/or power of attorney.

An exit Interview was held, and a copy of this report was provided to the facility.
