Facility Evaluation Report
On 05/29/2025, Licensing Program Analyst (LPA) Renee Campbell arrived unannounced regarding a staffing issue. LPA Campbell met with Administrator Jennifer Warkentin and explained the purpose of the visit.
A call was received by the Department regarding staff who work at the facility as a med tech who was underage. LPA Campbell requested a list of current staff with their birth dates, titles and the date of hire when they received their most recent titles. According to the staff roster and Guardian, staff member R1 was hired on 01/28/2025 as a Qualified Medication Aid (Med Tech). At the time of hire, R1 was 17 years and 7 months old. As of today's date, R1 is 17 years and 11 months old and thus still underage.
When discussed with Jennifer Warkentin, the administrator stated that since R1 had worked as a CNA in a skilled nursing facility, she had thought it was appropriate for R1 to work in this facility.
A deficiency was cited and R1 was removed from the staff roster and disassociated on Guardian as of 05/29/2025 by Administrator Jennifer Warkentin. LPA Campbell obseved the Electronic Personnel Action Notice (ePAN) as confirmation that R1 was separated from the facility.
Per California Code of Regulations, Title 22, the following deficiencies are cited during today's inspection.
Exit interview conducted and a copy of this report and appeal rights were left at the facility.
