Facility Evaluation Report

Licensing Program Analyst, LPA, Noel Wolf Petersen arrived unannounced to the facility to conduct a case management and deliver an amended report for a recent complaint. LPA met with the executive director Jackie Hernandez by phone and explained the purpose of the visit. LPA was informed Buisness Office Director Yaritza Alvarez would be the designated signatory for this report.

LPA arrived, read the amended report, provided explanation for its amendments and responded to immediate questions, encouraged any follow up questions by the facility to reach out by phone(619-323-4509) or email(noel.wolfpetersen@dss.ca.gov).
