Facility Evaluation Report

On 03/25/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA Pascua met with Facility Designated Administrator (FDA), Cami Azevedo and explained the purpose of this visit. The purpose was to conduct an annual.

There were two other staff members present, House Manager (HM), Maria Morton and Direct Support Staff (DSP), Wilder Diaz.

Current census was 4. A residents were out at their respective day programs at the time of this LPAs visit.

This facility is licensed to serve and retain 4 residents at any given time between the ages of 18 through 59. 2 out 4 residents may be non-ambulatory. This facility also is vendorized by Valley Mountain Regional Center (VMRC) to retain Level 6I residents at this time.

A brief interview with FDA Azevedo was conducted.

A review of 4 resident files were conducted. 4 out 4 files were complete and up to date. A review of 3 staff files were conducted. All files were complete and up to date. The FDA has an active certificate #70202549735 and expires on 10/08/2027.

The interior of the physical plant was in good condition and sanitary. Fire extinguishers appeared to have been annually inspected by Jorgensen company on 02/05/2026. The kitchen area was toured. LPA observed a sufficient seven days of non-perishable foods as well as two days worth of perishable food supplies in the main kitchen. Additional perishable and non-perishable food supplies were identified in the garage. Knives were observed to be locked under the kitchen sink and made inaccessible to the residents at this time.

LPA observed a locked centralized stored medication cabinet located in the hallway.
