Licensing Program Analyst (LPA) Jason Lund arrived unannounced to conduct a Case Management visit. LPA Lund met with Administrator Lorraine Padilla and explained the reason for the visit. Census 50
LPA Lund received an Unusual/Incident Injury Report from the facility on 9/16/2025 regarding Death Report for Resident (R1). LPA Lund could not review the UIR because the fax didn’t come out correctly. LPA Lund reviewed R1 was on hospice on 8/4/2025 by Seva Hospice and past away on 9/14/2025.
No deficiencies were cited during today's visit. Exit interview held and copy of the report and a copy of the appeal rights left.
