Facility Evaluation Report

Unannounced Case Management visit made out to this facility on 12/18/2025 by Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility designated Administrator Rosalinda Ortega. A brief interview was conducted with the facility designated Administrator at this time.

The purpose of this visit was to follow up on a request from this facility to change the mailing address at this time.

This LPA requested that the facility designated Administrator go ahead and contact the facility Licensee, Voica Matis, to inform her that CCL was present at this time.

This LPA was able to have a conversation with the facility Licensee over the phone and she stated that an updated form will be submitted into CCL for review by this LPA in order to change the mailing address for this facility.

Additional conversation was held with the facility Licensee in regards to the bedridden fire clearance and hospice waiver increase request at this time.

There were no deficiencies observed or cited during today's case management visit.
