Facility Evaluation Report
Unannounced case management visit made out to this facility on 09/22/2025 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility staff person, Pamela Hightower, and the facility designated Administrator, Ashlynn Burch, at this time.
A brief interview was conducted with the facility designated Administrator at this time.
The purpose of this visit was to follow up with resident R1 at this time. It was learned that R1 was recently placed here at this facility due to a change in R1's placement from R1's previous place of residence. A health and safety check was conducted by this LPA in regards to R1.
A brief tour of R1's bedroom was conducted.
A brief interview was conducted with R1 at this time.
There were no deficiencies observed or cited during today's case management visit.
