Unannounced Annual visit made out to this facility by Licensing Program Analyst (LPA) Jason Lund who was met staff and later with Administrator, Stella Hernandez. LPA Lund explained the reason for the visit to Administrator, Stella Hernandez. Census:5

LPA Lund & Administrator Stella Hernandez toured/inspected the facility. Inspection of the facility kitchen was conducted. Knives and sharps were locked and made inaccessible to clients. Cleaning agents, bleach, and other supplies were inaccessible to clients. A review of the facility's 2-day perishable foods and 7-day nonperishable foods was conducted and was sufficient. Medication cabinet, located in the facility office locked and inaccessible to clients. Policies and procedures involving handling, dispensing, and documentation of the resident medications was in compliance. Medication Administration Record and dispensing log was in compliance. Living room, dining area, and all other areas intended for resident use were observed to furnished and maintained in compliance at this time and able to meet the needs of the residents. Clients’ bedrooms were in compliance. Furniture and furnishings were observed to be sufficient and able to meet the needs of the residents at this time. Clients restrooms was incompliance. Laundry, garage was inspected and in compliance. First aid kits was observed and in compliance. Fire extinguishers were reviewed on 12/23/2025 observed to be in compliance. A tour of the exterior grounds was conducted and in compliance. A review of 2 staff and 2 clients was in complete and in compliance.

There were no deficiencies observed or cited during today's annual visit.
