Facility Evaluation Report
Licensing Program Analyst (LPA) Jason Lund arrived unannounced to conduct an one-year annual required inspection. LPA Lund was met by Administrator Keisha Jones and explained the reason for the visit. Census:4
LPA Lund & Administrator Keisha Jones toured/inspected the facility including the kitchen/ dinning area was inspected to make sure that the facility had a sufficient supply of kitchenware. The living area, and all other areas designated for resident use were observed to be furnished and maintained in compliance at this time. An inspection of the clients bedrooms & restrooms was conducted and able to meet the needs of the residents at this time in compliance. Food storage units were observed to be present and in functional order at this time. A inspection 2-day perishable and 7-day non-perishable food was observed to be in compliance at this time. Additional food storage units were observed to be in the garage and is compliance. Medication cabinet, located in the facility entry closet, was inspected. Medications were observed to be stored and made inaccessible to the residents at this time. A review of the facility Medication Administration Record was conducted at this time. First aid kit was observed to be present and did contain all of the required components at this time. Fire extinguishers, located throughout this facility, were checked by the local fire extinguisher company, on 10/22/2025 and in compliance at this time. A tour of the facility exterior grounds was inspected and in compliance. LPA Lund reviewed 2 facility staff & 2 client files was conducted and in compliance.
No deficiencies were observed during today’s visit. Exit interview and report left.
