Facility Evaluation Report

An office visit was held today, regarding the over-all func tion and compliance of the facility. Present at the meeting are Licensing Program Manager, Czarrina Camilon-Lee, Licensing Program Analyst, Albert Johnson and Jason Lund with Administrator Jenna Pamittan.

The purpose of this meeting is to discuss the on going issues with the facility and compliance with:

The facility has submitted plans of corrections for the above mentioned citations.

The Department will be taking the following actions:

Increased monitoring of the facility will take place.

Facility/Licensee will be referred to our Legal Division for review.

As a result of this conference a citation will be given for Administrator Qualifications
