The inspector’s account

Licensing Program Analyst (LPA) Jason Lund arrived unannounced to the facility to conduct a one year required inspection. LPA Lund met with Lead Manda Grubbs and explained the reason for the visit. Administrator Elizabeth Weil could not make the visit and gave permission to Lead Manda Grubbs to sign required paperwork. Census: 6

LPA Lund & Lead Manda Grubbs toured/inspected the facility including the common areas, the kitchen, bedrooms, bathrooms, and backyard areas. Furniture and furnishings were sufficient to meet the needs of prospective residents. Bedrooms contained a bed and mattress, nightstand, closet, chair and lamp. The indoor temperature was 78 degrees Fahrenheit (F), which is within the required range of 68 and 85 degrees. The home's water temperature measured 118 degrees F, which is within the required range of 105 and 120 degrees. Knives and cleaning products were locked under the kitchen cabinet out of reach of residents. LPA Lund observed first aid supplies that included thermometer, tweezers and scissors. There was fully-charged fire extinguishers (3/16/2026). Carbon monoxide/smoke detectors were tested and found to be fully functioning. LPA Lund observed a locked cabinet for the storage of medication in the hallway leading to the bedrooms. LPA Lund reviewed three staff & three residents files were in compliance at this time.

No deficiencies were observed during today’s visit. Exit interview was conducted with Lead Manda Grubbs and a copy of report left.
