Facility Evaluation Report
On 10/22/2025, Licensing Program Analyst (LPA) Renee Campbell arrived unannounced to conduct a post-licensing visit. LPA Campbell met with Patty Lynn Brooks, Administrator and explained the purpose of the visit. The licensee greeted LPA Campbell upon arrival. Upon entry, LPA Campbell observed a resident watching television and staff working in the kitchen.
The community is a one floor facility licensed to serve 6 residents age 60 and over. There may be 2 bedridden residents and there is a hospice waiver granted for 4. The facility also serves residents with dementia.
LPA Campbell requested the MAR and medications for residents in order to conduct an audit. Medications were found to be locked in a closet and inaccessible to clients. LPA Campbell reviewed the Medication Administration Record (MAR) and compared it to residents medication name and dosage, recipient name and time and frequency to be taken. After a review of medication for 2 of 5 residents, medications were found to be present and accounted for and accurately documented in the MAR. The first aid kit was inspected and found to be complete with a tweezer, thermometer, scissors and first-aid manual.
LPA Campbell also toured the outside of the community. A review of the perimeter fence, side gates, walkways were observed to be maintained in compliance at this time. Window screens were found to be intact. Pathways were clear and there were no pathway obstructions. There was shaded seating available for residents.
No deficiencies were observed or cited during this visit. Exit interview held and a copy of report left.
