Facility Evaluation Report

On 05/08/2025, Licensing Program Analyst (LPA) Renee Campbell arrived to the facility unannounced in response to the regional office being notified that the administrator and owner, Kelsey Ramos, had passed. LPA Campbell met with Linda Garrett and explained the reason for the visit.

LPA Campbell observed that the facility was free of odor and pathways were clear. There were 6 residents present in the facility. There were 2 staff present. LPA Campbell asked if the facility needed anything to help assist residents or if there was a lack of services available for residents. Staff Garrett stated no and that residents were doing well.

During the visit, LPA Campbell called Joshua Jacopetti by phone and he stated he was on his way to this facility to handle financial issues.

An exit Interview was held, and a copy this report was provided to the facility.
