The inspector’s account

On 05/19/2025, Sacramento South Regional Office staff, Assistant Program Administrator (APA), Pam Gill, and Licensing Program Analyst (LPA) Arielle Pascua arrived announced to this facility for the purpose to conduct a Health and Safety visit. LPA and APA met with House Manager, Cindy Fuentes and explained the purpose of the visit. Current census was 5. A brief interview with HM Fuentes was conducted.

Upon arrival, the facility was informed that the Department has appointed Robert Cantoria as the Temporary Manager, effective today 05/19/2025, who will assume responsibility for all operations of the facility. During the visit, facility representatives participated in an open discussion to address comments and concerns from family members.

A comprehensive tour of the facility was conducted to assess for any immediate health or safety risks to residents. At the time of the visit, two staff members were present on-site.

The food supply was reviewed and found to meet the required minimums: a two-day supply of perishable items and a seven-day supply of non-perishable goods. The kitchen refrigerator was inspected, and cabinets were observed to be stocked with non-perishable food items and dry goods. Medications were also reviewed.

Resident bedrooms were toured and found to contain adequate furniture and furnishings to meet residents' needs. Resident restrooms were inspected, and grab bars as well as non-skid mats were observed to be in place and in good condition.

The Department will conduct daily calls with TM Robert Cantoria to ensure ongoing compliance with Title 22 regulations. These calls will include updates on the resident roster, confirmation that residents are receiving appropriate care, and discussions regarding relocation plans for any residents who have been or may be relocated.

Per California Code of Regulations (CCR) -Title 22, Division 6, Chapter 8, no deficiencies were observed. An exit interview was held with HM Fuentas and a copy of this report was provided at the end of this visit.
