Facility Evaluation Report
On 03/27/25, Licensing Program Analyst (LPA) Renee Campbell arrived to the facility unannounced to conduct an annual facility inspection. LPA Campbell met with Mandy Witthar, Administrative Assistant and explained the purpose of the visit. Upon entry, LPA Campbell observed two clients in the facility. Later, two more clients returned from their outing. They were observed watching TV, returning to their rooms or getting a snack.
The facility is a two story building licensed to serve 6 ambulatory, developmentally disabled clients between the ages of 18 and 59. Referrals are obtained from Valley Mountain Regional Center (VMRC). Prior citations have involved missing screens and unsecured medication. During the facility tour, screens were intact and medications were observed to be secured by a fingerprint lock for staff.
LPA Campbell toured the facility and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas to ensure there are no safety hazards for residents.
Furniture and furnishings were sufficient to meet the needs of residents. Client bedrooms were observed to be in good repair with the required accessories of bed, lamp, chair, nightstand and closets.
