Facility Evaluation Report
Licensing Program Analyst (LPA) Jason Lund arrived unannounced to conduct a required Annual Inspection. LPA Lund was allowed entry into the facility by staff. LPA Lund spoke with Administrator Stacy Wycoff who could not make today’s visit. She gave Facility Manager Melinda Carrillo permission to sign required paperwork.
LPA Lund and Facility Manager Melinda Carrillo toured/ inspected the physical plant inside and outside to ensure all passageways, and other areas of potential hazard are free of obstruction. LPA observed the kitchen and dining area for the ability to prepare food. LPA observed bedrooms and bathrooms, storage areas, laundry and lighting throughout the facility. The hot water temperature was measured at 107*F which is within the required range of 105-120*F. LPA observed the 2 -day perishable and the 7- day nonperishable during this visit.
LPA observed the fire extinguisher(s), carbon monoxide, smoke detectors ,fire extinguishers (10/17/2024) and first aid kit were observed to be in compliance. Facility has central heating and air. Staff and resident files were readily available for review during this visit. LPA reviewed 2 Client Records, and 2 Staff Records Staff were reviewed. All staff record records reviewed provided documentation that staff had required documentation. All required Client documentation was in each file.
Based on today's inspection conducted by LPA Lund, no deficiencies are being cited.
