Facility Evaluation Report
On 09/30/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA was greeted by Sharmari Gonzalez and explained the purpose of the visit. It was learned that Facility Designated Administrator (FDA), Latrice Gonzalez was unable to meet with LPA Pascua at this time and appointed Sharmari Gonzalez to complete the visit at this time.
The purpose of this visit was to conduct annual visit.
This facility is currently awaiting vendorization at this time and there are no current residents in care.
A tour of the facility was conducted to ensure that facility safety was in compliance, such as the fire extinguishers, smoke detectors, and carbon monoxide were present. All were present and in good repair at this time. A tour of facility bedrooms were conducted to verify that there are currently no residents in care.
LPA advised Shamari Gonzalez to inform FDA Gonzalez to notify the department once facility obtains vendorization and places residents.
Based on the observations made, there are no deficiencies observed during this annual visit.
An exit interview was conducted and a copy of this report was provided to the facility.
