Facility Evaluation Report
Licensing Program Analysts (LPA's) Renee Campbell conducted an unannounced required 1 year annual inspection on 07/29/2025. LPA Campbell met with Administrator Clayton Forrest, COO Jennifer Campbell and CEO Carl Wood and explained the purpose of the visit.
LPA Renee Campbell reviewed five resident files and three staff files. All staff were fingerprint cleared and have current first aid certificates. LPA's toured the facility with the Administrator and inspected classrooms, the kitchen, bathrooms, storage and property. Furniture and furnishings were sufficient to meet the needs of clients. The facility temperature was 70 degrees Fahrenheit, which is within the required range of 68 to 85 degrees.
LPA Renee Campbell observed first aid supplies, a fully charged and up-to-date fire extinguisher dated 07/29/2025. Per Administrator Forrest, the day program provides no meals or snacks.
No deficiencies were cited during this visit. An exit interview was conducted and a copy of this report was left with Clayton Forrest.
