Facility Evaluation Report
Licensing Program Analyst (LPAs) Renee Campbell arrived at the facility to conduct an unannounced annual inspection on 08/07/2024. LPA Campbell met with Gabriel Deubert, Transitional Program Director and explained the purpose of the visit.
LPA inspected the physical plant including classrooms, break out rooms, bathrooms, kitchen and staff rooms to ensure they met with Title 22 regulations . This day program resides in the basement of a one story building and is licensed to serve 70 ambulatory residents of which 10 may be non-ambulatory. Per the facility and Department records, there is a rider regarding the day program in person capacity. On average, there are 50 to 55 clients in person in the facility. Today, the facility has a census of 53. This does not include client’s who work and receive coaching in the community. LPA observed classrooms and bathrooms to be free of odor, clean and in good repair. There are no bodies of water present .
LPA Campbell observed the location of the first aid kit in the staff room. Fire extinguishers, smoke and carbon monoxide detectors are in good repair and are connected to the FIre Department. The fire extinguisher was last inspected 08/10/2023. Facility thermostat observed at 72 degrees Fahrenheit. Medication is stored for one client for use at lunch . LPA Campbell reviewed 3 of 15 stafff files. Of the 72 client files, 7 were reviewed. Three of the three staff files reviewed were missing signed Employe Rights forms.. One of the seven client files reviewed was missing an Admission Agreement. Toxins are made inaccessible to clients in care and locked in a janitor's closet.
