On 9/11/25 Licensing Program Analyst (LPA) Triel Ellen Lindstrom and Licensing Program Manager (LPM) arrived unannounced at the facility to conduct a case management visit. The LPA met with the Administrator and explained the purpose of the visit.
During the tour of the facility, LPM Rios was in room 27 and noticed that the call button doesn't work. Residents stated that it hasn't worked in at least a week. Having an inoperable emergency call button can cause a potential risk to residents in care.
Exit interview was conducted and a copy of the report and appeal rights was left at the facility.
