Facility Evaluation Report

LPA arrived to the facility 7am on 3/27/26 to conduct an annual visit. After knocking on the door, LPA noticed the doorbell disabled and a pile of broken bricks in the front yard. LPA met with Administrator Genaro Baisac Jr. by phone and was told:

the previous residents in care were moved another facility operated by the administrator(Valley Royale Care, 507002686),

The department will follow up with the CHOL application in the near future, probably next week. LPA gave guidance that abandoning the property generally constitutes a forfeiture of the license, very likely a new application will need to be submitted.

Report was read to the administrator over the phone, an exit interview was conducted. As the administrator is currently out of town and there is no staff to sign or physcially receive the report, the LPA will send a copy of the report to the email address on file with a send back request for the signature.
