Facility Evaluation Report

Licensing Program Analyst, LPA, Noel Wolf Petersen arrived to the facility 10:30am to collect some documents related to an investigation. Licensing Program Analyst, LPA Noel Wolf Petersen met with the staff Noemi Tlatlepa and Adrian Garcia and later Assistant Program Director Nathan Ayala and Program Director Tamra Hernandez to explain the purpose of the visit.

LPA collected a copy of a LIC 500, LIC 9020, PreAppraisal functional capability assessment and 603, 602 from program entry, IPP, admission agreements, and relevant SIRs.

Clients are responding positively to questions about thier care. Staff are responding positively to questions about their responsibilities.

A walk through of the facility was conducted, facility is clean and in good repair.

A copy of the report was read and given to the administrator. Exit interview was conducted.
