The inspector’s account

Licensing Program Analyst Noel Wolf Petersen, arrived to the facility unannounced, to conduct an annual inspection. The LPA met with the administrator Tamera Hernandez and Assistant program Director Nathan Ayala to explain the purpose of the visit.

Physical plant inspection included but was not limited to the the exterior, evacuation route, common areas, smoking area, activity classrooms, break room, office spaces, and storage areas. The facility has lockable storage areas in the kitchen, the evacuation route gate swings freely and latches closed, common areas and exteriors are free of trip hazards. The facility is clean, furniture and furnishings are in good repair, the traffic areas are well lit and unobstructed.

The fire extinguishers are dated 12/4/25 and checked for charge by the facility every month. first aid kit has a all necessary items. Smoke alarm is functional.

5 staff records were reviewed for initial training's, continuing training's, health screenings, and background clearance, the documents are present and up to date. 5 staff were interviewed regarding thier abuse training, personal rights training, and internal documentation process.

5 clients records were reviewed for the admission agreement and individual needs and services plans, and clients are up to date.

Administrator docs were reviewed including the evacuation procedure, and infection control plan, administrator certificate, and hiv training. documents are present and up to date.

no deficiencies were cited as part of this visit, A copy of the report was read and given to the administrator, an exit interview was conducted.
