Facility Evaluation Report
Licensing Program Analyst (LPA) Renee Campbell arrived announced to conduct a pre-licensing inspection due to a change of location. LPA Campbell met with Valley Caps Program Director Tamra Hernandez, and explained the purpose of the visit. The facility will be moving to a new location at 108 Campus Way, Modesto, CA from 1900 Blue Gum Ave, Suite B, Modesto, California 95358. The new location has been cleared for 150 ambulatory clients and 24 non-ambulatory clients.
Upon arrival, LPA Campbell observed 3 staff members and 0 participants. All current participants are attending Day Program at the previous active location. LPA Campbell toured the physical plant to ensure compliance with Title 22 regulations. LPA Campbell inspected 5 classrooms, 3 offices rooms, 4 multi-function rooms, a changing room, the kitchen, a Gym Hall and 7 bathrooms with multiple stalls. There are 11 fire extinguishers throughout the building. The last inspection for the fire extinguishers was completed on 12/05/2024. A locked cabinet for knives was observed. There will be areas for participants to place their belongings in lockers observed in the Gym hall . When needed, clients will have a secured area for their medications in the Med Room along with the first aid kits and AED's. The Gym area will serve as a multi-purpose room for additional classes, meals, movies and other entertainment. The program has an infection control plan and an emergency disaster plan in place that was reviewed and approved by the Regional Office. Water temperature was measured at 114 degrees Fahrenheit (F) as required. Per California Code of Regulations (CCR) - Title 22, no deficiencies were observed. Comp III was completed and passed. LPA Campbell has no objections to licensure. This prelicensing inspection has been passed. An exit interview was conducted and a copy of this report was left with Tamra Hernandez, Program Director.
