Facility Evaluation Report
On 9/23/2025, Licensing Program Analyst (LPA) Triel Ellen Lindstrom arrived at the facility unannounced to conduct a required 1-year annual inspection. LPA Lindstrom met with Administrator Marisol Moreno and explained the purpose of the visit.
The facility is a day program that currently serves fifty-three clients. There were thirty-nine clients in the program on the day of the site visit. LPA Lindstrom toured the interior of the facility. The interior was clean, orderly, and odor-free. The walkways were free of obstructions. One side of the facility contained administrative offices, and the other side contained program rooms and areas for clients, including a main hall, classrooms, a kitchen, changing rooms, and bathrooms. The main hall was well lit and had adequate seating and space for clients. The room contained décor on the wall and storage lockers for clients. The bathrooms were large, accessible, and had grab bars. The bathroom hot water temperature was 116 degrees Fahrenheit, within the required range. Toxins, cleaners, and sharps were stored in a locked storage room off the Main Hall. Medication was kept in a lock box in a staff office. There were complete, up-to-date records for medication administration and prescription orders for as-needed medication. The smoke detectors and sprinklers were an interconnected system that is inspected every five years and was last inspected on 11/8/2021. There were multiple fire extinguishers located throughout the facility, which were last serviced on 1/9/2025 by Joregensen Co. The carbon monoxide detector was located in the kitchen and in working order. The first aid kit contained the required items. LPA Lindstrom reviewed four staff files and five client files and found them organized and complete.
As a result of this inspection, no deficiencies were cited. The facility was in compliance with the California Code of Regulations (CCR), Title 22, Division 6. An exit interview was conducted with the Administrator, to whom a copy of this LIC 809 report was provided. Their signature below confirms receipt of this document.
