Facility Evaluation Report
On 11/18/2025, Licensing Program Analyst (LPA) Renee Campbell arrived to the facility unannounced to conduct an annual inspection. LPA Campbell met with Deborah Mensonides, Lead Caregiver and explained the purpose of the visit. Upon entry, LPA Campbell observed one resident seated at the dining room table and another resident speaking on the phone making plans for Thanksgiving with their family.
The community is a single story house licensed to serve developmentally disabled ambulatory adults age 18 to 59. Non-ambulatory adults would not be accepted. The physical plant consists of 4 bedrooms, 4 bathrooms and one office space. During a tour of the community, resident bedrooms were found to be furnished appropriately with a bed, chest, night stand, lamp and closet. LPA Campbell observed the locked medication storage in the dining room and there was sufficient lighting throughout the facility. The community was free of odor and clean. No bodies of water were observed.
An orange tree was observed in the backyard where pathways and exits were clear. Fire extinguishers were last inspected on 12/12/2024 and were fully charged. The facility temperature was 74 degrees which is within the required range of 68 and 85 degrees Fahrenheit. A shed in the backyard was opened and various backyard equipment was observed. In the garage, additional food is stored in a refrigerator and excess cleaning supplies were locked and inaccessible to residents. There is also an ExpressMix Chemical Dispenser which will take the place of all other cleaning products in the future.
In the kitchen, LPA Campbell observed a minimum 2-day supply of perishable food and a minimum 7-day supply of nonperishable food. LPA Campbell reviewed 4 of 4 resident files and 6 of 6 staff files and all were found to be complete. All staff were also found to be finger print cleared as well.
Per California Code of Regulations (CCR's) - Title 22, Division 6, Chapter 6, no deficiencies are being cited.
