On 10/9/25 at 11:00am Announced Office Meeting was conducted on 09/12/2025 via Teams Meeting by Licensing Program Manager (LPM) Lisa Rios and the following individuals as well:

The purpose of this meeting was to conduct an Office Meeting and follow up with the facility

Licensees, Esther and Patrcik Kalu, in regard to some recent events and incidents that have taken place at

this facility. The following are the areas of concern investigated by VMRC:

During an unannounced visit and follow up to an alert on 9/25/2025, the following areas did not meet compliance.

1. Medications Errors, inconsistency of medication administration, attempts to mislead a CSL from discovering medication errors during a walk through and improper storage of medications.

2. Residents are not allowed to access food without staff permission, not following house menu, lack of variety of food options for residents, no fresh vegetables, and no variety of fresh fruit.
