Facility Evaluation Report
LPA Campbell arrived unannounced regarding R2 who is currently incarcerated. LPA was met by Danyell Williams, Caregiver and explained the purpose of the visit. Upon entry LPA Campbell observed no residents in the common areas. There were two at day program and one resident still sleeping in his room. R2 is currently still incarcerated, with a court hearing on the 25 th of June at 8:30 am in Department 7 in Modesto at 800 11 th Street.
During the visit, LPA Campbell reviewed the UIR’s that had been sent to the Department as well as the Daily Progress Notes for R2 found in their binder kept at the facility. LPA Campbell also attempted to contact Social Worker Ophelia Nguyen and Service Coordinator, Khrysten Kagan. Voicemails were left but no calls were returned at this time. Caregiver Williams was also interviewed regarding R2 though she was not present for all events involving R2. LPA Campbell was also able to update contact information for the house phone and administrators cell phone.
Per the administrator, R2 will not be coming back and R2 verbally told the facility that they did not want to return but there is nothing in writing. For now, the facility has packed his things away but kept the items in the room and R2 is not officially evicted. LPA Campbell will need to further verify R2’s status with the facility with the service coordinator, social worker and/or licensee. No deficiencies have been cited at this time.
