Facility Evaluation Report

Licensing Program Analyst (LPA) Renee Campbell arrived unannounced to conduct an annual inspection. LPA Campbell met with Administrator Catherine Torrez and explained the purpose of the visit.

As observed by LPA Campbell, there was only one client present in the facility. The other three residents were at day program. Their client files indicate that all the residents are ambulatory

LPA Campbell toured the facility and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas to ensure there are no safety hazards for residents. Furniture and furnishings were sufficient to meet the needs of residents. The facility is currently renovating a facility bathroom. The bathroom is kept locked when construction is not in progress. However, there are an additional two bathrooms available for use for guests, staff and residents . The licensee has provided keys for all doors and outbuildings on the facility property.
