Facility Evaluation Report

LPA Jason Lund arrived unannounced to complete a case management visit. LPA Lund met with Administrator Steve Bryant and explained the purpose of today's visit.

Community Care Licensing received a Unusual incident/ injury report dated 9/1/2023 stating that client (C1) had an medication error and went to the hospital. C1 received a different clients medications and sent to the hospital. C1 is still in hospital. The facility had training (Six rights of medication training) with all staff who handle medications.

Deficiencies cited on the attached 809D. Report & appeal rights given.
